If you have any questions or concerns regarding the collection of this information, please contact REOS Academy. All information provided is treated as confidential and will not be shared with any third party, except where required for reporting to the national statistical database to support future federal funding for vocational education and training.

From January 1, 2015, REOS Academy will be prevented from issuing you with a nationally recognised VET qualification or statement of attainment when you complete your course if you do not have a Unique Student Identifier (USI). In addition, we are required to include your USI in the data we submit to NCVER. If you have not yet obtained a USI you can apply for it directly at http://www.usi.gov.au/create-your-USI/ on computer or mobile device. Please note that if you would like to specify your gender as ‘other’ you will need to contact the USI Office for assistance.

Your Unique Student Identifier (USI) is your personal education number required for all nationally recognised study, including university and vocational education and training (VET). Find yours here.

Why we collect your personal information

As a registered training organisation (RTO), we collect your personal information so we can process and manage your enrolment in a vocational education and training (VET) course with us.

How we use your personal information

We use your personal information to enable us to deliver VET courses to you, and otherwise, as needed, to comply with our obligations as an RTO.

How we disclose your personal information

We are required by law (under the National Vocational Education and Training Regulator Act 2011 (Cth) (NVETR Act)) to disclose the personal information we collect about you to the National VET Data Collection kept by the National Centre for Vocational Education Research Ltd (NCVER). The NCVER is responsible for collecting, managing, analysing and communicating research and statistics about the Australian VET sector.

We are also authorised by law (under the NVETR Act) to disclose your personal information to the relevant state or territory training authority.

How NCVER and other bodies handle your personal information

NCVER will collect, hold, use and disclose your personal information in accordance with the law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act. Your personal information may be used and disclosed by NCVER for purposes that include populating authenticated VET transcripts; administration of VET; facilitation of statistics and research relating to education, including surveys and data linkage; and understanding the VET market.

NCVER is authorised to disclose information to the Australian Government Department of Employment and Workplace Relations (DEWR), Commonwealth authorities, state and territory authorities (other than registered training organisations) that deal with matters relating to VET and VET regulators for the purposes of those bodies, including to enable:

  • administration of VET, including program administration, regulation, monitoring and evaluation
  • facilitation of statistics and research relating to education, including surveys and data linkage
  • understanding how the VET market operates, for policy, workforce planning and consumer information.

NCVER may also disclose personal information to persons engaged by NCVER to conduct research on NCVER’s behalf.

NCVER does not intend to disclose your personal information to any overseas recipients.

DEWR is authorised by law, including the Privacy Act and the NVETR Act, to collect, use and disclose your personal information to fulfil specified functions and activities.

Surveys

You may receive a student survey which may be run by a government department or an NCVER employee, agent, third-party contractor or another authorised agency. Please note you may opt out of the survey at the time of being contacted.

Consent for publication of photographs and student work

  • REOS Academy occasionally takes photos ofstudents participating in classesfor publicity purposes. These photos may be displayed on our website. The names and details of the people in the photos are not released or published. Staff will alwaysidentify when they are taking photos so students who don’t wish to have their photo taken can be excluded from the photo. If at any time your photo is published on the website and you would like it removed, we will do so within 24 hours of receiving a written request to remove it.

Consent/authority to release information and view documents

Please be assured that any discussions held with this representative will be for the purposes of your assessment and for your skills development.

During the process we do not plan to discuss your evidence or work practices with other trainees, unless we have your written permission to do so.

You are required to give permission in writing for any of these discussions or viewing of evidence to occur.

  • I will be required to participate in the completion of a National Students Outcomes Survey [NCVER], during the course of my training program.

Declaration of Information Accuracy

In signing or emailing this form I acknowledge and declare that;

  1. I have read and understood and consent to the privacy notice and have completed all questions and details on the enrolment forms.
  2. Arrangements have been made to pay all fees and charges applicable to this enrolment.
  3. I have read and understand REOS Academy’s Student Handbook
  4. I agree to be bound by the REOS Academy’s Student Code of Conduct, regulations, policies and disciplinary procedures whilst I remain an enrolled student.
  5. I am 18 years of age or older.
  6. My participation in this course is subject to the right of REOS Academy to cancel or amalgamate courses or classes. I agree to abide by all rules and regulations of REOS Academy.
  7. I understand and have been provided with information by REOS Academy in relation to Credit Transfer and Recognition of Prior Learning (RPL).
  8. I confirm that I have been informed about the training, assessment and support services to be provided, and about my rights and obligations as a student at REOS Academy.
  9. I have also visited REOS Academy’s website to review Training and Assessment options available to me including but not limited to duration, location and mode of delivery, fees, refunds, complaints and withdrawals.
  10. I authorise REOS Academy, in the event of illness or accident during any REOS Academy organised activity, and where emergency contact next of kin cannot be contacted within reasonable time, to seek ambulance, medical or surgical treatment at my cost.
  11. My academic results will be withheld until my debt is fully paid and any property belonging to REOS Academy has been returned.
  12. I acknowledge that from time to time REOS Academy may send me information regarding course opportunities and other promotional offers and that I have the ability to opt out.
  13. I declare that the information I have provided to the best of my knowledge is true and correct.
  14. I consent to the collection, use and disclosure of my personal information in accordance with the Privacy Notice above
